Still have issues with your password and logging into modernized Trails?
Please contact the Help Desk at 303-239-4357, when prompted press 1 then press 3.

Please be sure to identify yourself to the help desk representative as: “I’m a Licensed Public Provider, and I need my CDHS Portal Password Reset so I can log into Trails Mod”.
